The Governance Gap Behind the EBP Boom

Emerging biopharma is driving more clinical trial starts than ever before.  But too many programs are being asked to scale scientific ambition without the operational architecture required to carry it.

Emerging biopharma companies now account for 68% of all clinical trial starts, up from 56% in 2019.   That is an extraordinary signal of innovation, velocity, and confidence in the science.

It is also a warning.

The greatest risk to many emerging biopharma programs is not that the science will fail.  It is that the operating model will fail the science.

Lean teams are often described as agile.  Sometimes they are.  But a lack of governance will always stand in the path of that agility as unmanaged ambiguity.

When decision rights are unclear, issues sit too long.  When escalation pathways do not exist, risks arrive at the executive level only after they have become timeline, budget, enrollment, quality, or vendor-performance problems.  When there is no integrated view of clinical operations, finance, regulatory, data, and CRO delivery, each function may be working hard while the program as a whole drifts off course.

A development program rarely fails because no one cared.  It fails because no one had a reliable mechanism to see, decide, escalate, and act before the problem became irreversible.

This is where operational excellence matters.

Operational excellence is creating the minimum viable infrastructure needed to make good decisions quickly, manage tradeoffs deliberately, and deliver reliably under pressure.

For an emerging biopharma sponsor, that infrastructure often includes:

  • Clear program-level decision rights and accountable executive sponsorship

  • A practical PMO or program-management backbone

  • Defined escalation thresholds for timeline, quality, budget, enrollment, and CRO risks

  • A disciplined sponsor–CRO governance cadence focused on decisions (not status reporting)

  • Integrated visibility across milestones, spend, risk, and delivery dependencies

  • Leading operational indicators that identify drift before a milestone fails

Scientific uncertainty is unavoidable.  Development programs will encounter unexpected safety signals, enrollment challenges, site performance issues, protocol amendments, shifting capital constraints, and vendor complexity.  But uncertainty does not have to become chaos.

The organizations that understand how decisions get made, who is accountable, what triggers escalation, and how quickly the team can move from signal to action have the opportunity to achieve operational excellence.

In other words:  they are the ones that give the science an operating model worthy of its ambition.
