AI is accelerating pharmacovigilance by automating adverse event intake, case triage, coding and duplicate detection, and by supporting signal detection across large datasets. Because safety data directly affects participants, this is also where sponsors must be most rigorous about human review, validation and oversight.
Safety case processing has always been volume-heavy and deadline-driven. Expedited reporting timelines leave little room for delay, and case volumes grow as programs expand. It is easy to see why automation is attractive.
How is AI used in drug safety?
Case intake. Natural language processing extracts key information from source documents, emails and forms, populating case fields that were once entered manually.
Triage and prioritization. Models can flag cases likely to be serious or unexpected so they reach a qualified reviewer first, which helps protect expedited reporting timelines.
Coding. Auto-coding of adverse events to MedDRA terms, with human review of uncertain matches, is now routine at many safety vendors.
Duplicate detection. AI can identify likely duplicate reports across sources, a tedious manual task.
Signal detection. Machine learning can analyze aggregate data to surface potential safety signals earlier than traditional disproportionality methods alone.
Why must humans stay in the safety loop?
Causality assessment, seriousness determination and expectedness judgments require medical expertise and context. A model can suggest. A qualified physician must decide. An error in triage that delays an expedited report is not only a compliance problem. It can mean a safety signal reaches investigators and regulators late.
Sponsors should confirm that their safety vendor's AI outputs are reviewed by appropriately qualified staff before any regulatory decision is made, and that the level of review is proportionate to the case's seriousness.
What sponsors should ask safety vendors
Which steps in case processing use AI, and at what confidence thresholds is human review triggered?
How was each tool validated, and how is ongoing accuracy monitored?
What happens when the model encounters an unfamiliar document format or language?
How are model updates controlled and communicated?
Can you show the audit trail for an AI-assisted case from intake to submission?
Small sponsors are particularly exposed
Emerging sponsors typically outsource safety entirely. That makes vendor oversight the only safeguard. Include safety AI in your vendor oversight plan, review reconciliation between the clinical and safety databases, and periodically sample processed cases yourself or through an independent reviewer.
AI can make pharmacovigilance faster and more consistent. The safety standard has not moved, and neither has the sponsor's responsibility to meet it.
