All three models can work. And all three can fail expensively.
A full-service CRO can bring integrated execution when sponsor capacity is lean. But it does not remove the sponsor’s responsibility to make clear, timely decisions.
An FSP model can provide flexibility and control. But without strong sponsor-side functional leadership and cross-functional integration, it can quickly become a collection of well-staffed silos.
A hybrid model can be powerful. It can also turn into a maze of handoffs, duplicate work, and “I thought they owned that” moments.
The most important question a biotech or emerging biopharma sponsor can ask (and answer): Which model fits this program, this company, and this moment?
That requires an honest assessment of:
Trial complexity and development objectives
Internal expertise, capacity, and leadership bandwidth
Decision rights and governance maturity
Appetite for control, variability, and delivery risk
The sponsor’s ability to oversee CRO work
When the model and sponsor capability are misaligned, the symptoms are predictable:
Change orders become the operating plan.
Governance meetings become reporting rituals.
Escalations arrive after options have narrowed.
Timelines slip while accountability circulates.
The strongest CRO partnerships are designed through a sponsor-side system that defines accountability, decision-making, performance expectations, and escalation before the pressure arrives.
